| Application Form Please
take note of the following instructions before moving on to the module allowing
you to fill in your visa application form online.
After having filled
in your visa application form, a printable form, in the PDF file format, will
automatically be generated with your personal data. You will be invited to download
this file on your computer, to print it and to sign it. If you are unable to read
this file, you will need to download the software Acrobat
Reader.
Once your visa application form is filled online, you will
have 15 days to submit it to us. After this period, your personal data will be
erased for security reasons and you will be required to fill in the form online
once again.
Important Information
For security reasons,
you may only register your passport number once. For this reason we request you
to note down your Web Reference Number as soon as it appears on the screen. This
number begins with W followed by a randomly generated number (e.g. W010108123456789).
It will not be possible for you to access your application details without this
number. You would then be obliged to contact our call centre to retrieve your
Web Reference Number after an identity check.
Instructions for
filling in the online application form - You should answer all
questions. Answers should be written in English.
- Letters with foreign
symbols/accents are not accepted. E.g. Monsieur André Du-Château
should be entered as follows : Andre Du Chateau
- The << Download
Application Form >>button allows you to download and then print your application.
Please do not forget to sign your form once printed.
Frequent
Errors - Your name must be reproduced exactly as it is on
your passport
- All first names should be entered in the field
marked << First Name >>
- All surnames should be
entered in the field marked <<Last Name >>
- The field
<< Middle Name >> should not be used
- The term <<
State >> is the equivalent to << Département >>in French;
the term << DOB >> means << Date of Birth >>.
- Your
telephone number (preferably a mobile) should be filled in the 3 boxes as per
the following example: 0033 6 00000000.
